Participants should complete this Chronic Condition Management Program at Flinders University with the following capabilities including be able to:Demonstrate understanding of the concepts and principles of the Flinders Chronic Condition Management ProgramIdentify the principles and processes of Motivational InterviewingIdentify first steps to implement chronic condition management with self-management supportUsing this knowledge, participants should have the capacity to:Assess a client’s self-management capacity using the Partners in Health Scale and the Cue and Response interviewWork with a client to identify a significant problem and link mid- to long-term goals using the Problems and Goals AssessmentAssist a client to problem solveUse behaviour change counselling skills to maintain a client centred focus throughout the interviewDevelop an evidence-based behavioural care plan to improve and support self-management of the client’s chronic condition(s).Programme Structure
Course content- This short course introduces the structured process and set of tools underpinning the Flinders Program approach to self-management support and collaborative care planning. The program includes supervised practice of self-management assessment and care planning.
