This Flinders Accredited Trainer Program short course will enable participants to train others in the Flinders University Chronic Condition Management Program. By training others in the Flinders Program, participants can contribute to the broader support of self-management.

Overview

Learning outcomes

The participants who complete the Flinders Accredited Trainer Program will be able to:
  • Plan, evaluate and report on Flinders Program Training
  • Meet adult learning principles and program standards as defined in the Flinders University Program license agreement.
  • Provide specific CCM skills training including review and feedback on:
  • Outlining this approach to assist with identifying patient’s capacity to make health behaviour changes
  • Completing self-management assessments
  • Collating assessment information into a self- management Care Plan
  • Tailor learning activities to support various learning styles
  • Support health practitioners to identify how to manage change supporting CCM in their practice
  • Support health practitioners to develop their ability to collaborate with clients to better manage chronic conditions

  • A 2-day workshop preparing participants to train others in the Flinders Program. This offers the opportunity to contribute to workforce, system and organisational capabilities in self-management and self-management support in chronic condition management (CCM).
