Learning outcomes
The participants who complete the Flinders Accredited Trainer Program will be able to:- Plan, evaluate and report on Flinders Program Training
- Meet adult learning principles and program standards as defined in the Flinders University Program license agreement.
- Provide specific CCM skills training including review and feedback on:
- Outlining this approach to assist with identifying patient’s capacity to make health behaviour changes
- Completing self-management assessments
- Collating assessment information into a self- management Care Plan
- Tailor learning activities to support various learning styles
- Support health practitioners to identify how to manage change supporting CCM in their practice
- Support health practitioners to develop their ability to collaborate with clients to better manage chronic conditions
Programme Structure
Course content- A 2-day workshop preparing participants to train others in the Flinders Program. This offers the opportunity to contribute to workforce, system and organisational capabilities in self-management and self-management support in chronic condition management (CCM).
